This quote comes from a study published in Physical Medicine and Rehabilitation in 2004 entitled Effects of a late-phase exercise program after total hip arthroplasty: a randomized controlled trial:

“An exercise program emphasizing weight bearing and postural stability significantly improved muscle strength, postural stability, and self-perceived function in patients 4 to 12 months after [Total Hip Arthroplasty] THA.”

This quote, from a study published in the Journal of Experimental Orthopaedics in 2018 entitled Rehabilitation strategies for optimisation of functional recovery after major joint replacement, concludes that doing exercise after hip replacement surgery is superior to not doing it.

“...post-operative, exercise-based, rehabilitation is superior to no or minimal rehabilitation after Total Hip Arthroplasty (THA)”

Over the last 25+ years, I’ve had hundreds of clients come to me because they were struggling with their recovery after hip replacement surgery.

...but they were still stuck!

After reviewing things with a number of these clients, I finally figured out what was happening.

When I saw the exercise programs my clients received from their surgeons, doctors, and physiotherapists, I was shocked!

In most cases, it was a single sheet of paper that looked like it was the 10,000th photocopy from something that was first produced in the 1970s.

A lot of the time, there weren’t even photos, but hand-drawn illustrations of how to perform the exercises.

If these exercises were based on research, they had to be researched from decades ago.

And, there was little to no instruction or guidance included.

I couldn’t believe it!

Especially considering how important exercise is to a patient who has just had major surgery to replace a major joint in their body.

I was appalled... because this level of patient care was reprehensible.

But then I figured out why this was happening...

Doctors and surgeons are trained in diagnosis, treatment and surgery.

(1) CARDIOVASCULAR

After your hip replacement surgery, many parts of your body will seem stiff and tight. This is normal, but we want to get your body loosened up and get your cardiovascular system going before we start the other exercises and movements. This will make everything that follows easier and more effective. We’ll do some light walking to warm up before we do each exercise routine.

(2) RANGE OF MOTION

After hip replacement surgery, most people will have a limited range of motion in their hips and the surrounding areas. We’ll do a variety of different exercises, including seated and standing marches, side shuffles, leg abductions, leg extensions, and more, to increase this range of motion so you can easily move your body however you need to again.

(3) STRENGTHEN HIP FLEXORS

The hip flexors—a group of muscles that allow you to lift your leg and knee, are significantly weakened after hip replacement surgery due to the incision made during surgery, and the swelling and inflammation that occurs after surgery. Strengthening your hip flexors is a crucial step in re-stabilizing, supporting and strengthening your replacement hip after surgery.

(4) SIDE-TO-SIDE

After a hip replacement surgery, you will be weaker and limited in your side-to-side range-of-motion and balance. Correcting this weakness is crucial because most people who fall after a hip replacement surgery do so because of weakness or poor balance in the side-to-side direction. In the program, we’ll work on strengthening your side-to-side movement, increasing your side-to-side range of motion, and improving your side-to-side balance and stability.

(5) BALANCE

After hip replacement surgery, your whole body balance changes, because you’ve made a change in one of your largest joints, so this has an effect on your overall balance. In the program, we’ll work on hip stability, single-leg balance, uneven surface balance, and more. Don’t worry, every exercise we do is completely safe, because they are all designed specifically for hip replacement patients who have balance limitations.

(6) STRENGTHEN GLUTES

The gluteus maximus and medius muscles help stabilize the hips, help with hip extension, and transfer power to the legs through the knees. Since these muscles support the hips so much and help with so many different movements, it is crucial to strengthen them after hip replacement surgery, so you don’t lose range of motion and mobility in your hips, knees and legs.

(7) STEP UPS

One of the common movements that will be impaired after your hip replacement surgery is walking up stairs or steps. So in the program, we’ll work on strengthening the area around your hips and the areas around your feet, ankles, knees, core and pelvis. And then we’ll do some gentle stepping exercises, so that we can fully restore your ability to walk up and down steps.

(8) STRENGTHEN KNEES

After your surgery and during the recovery period, your knee on the operated leg will be weaker than usual. Since the knee helps support the hip in many different lower body movements, we’ll work on strengthening the knee so it can be restored to full power and provide your hip with all the support it needs.

(9) SITTING TO STANDING

Another common movement that is affected by hip replacement surgery is moving from a sitting position to a standing position. In the program, we work on this movement gradually and gently, so you stay comfortable and safe, but slowly progress to restoring this all-important functional movement.

Heat and ice are a very important part of recovery, and we give you specific instructions so you can reduce inflammation and pain, and get back to your normal life as quickly as possible!
